New patients are asked to fill out any appropriate forms below before you come to the office and to arrive 15 minutes before your appointment time. This time will be used to verify your information and answer any questions. We also kindly ask that any cancellations be made at least 48 hours in advance to avoid a cancellation fee ($75/hr).
